WRAPAROUND PROGRAM
POSITION: INTENSIVE CARE COORDINATOR
LOCATION: WESTERN MASSACHUSETTS (SPRINGFIELD)
DEFINITION: The ICC, using the principals and guidelines set forth by the Children’s
Behavioral Health Initiative, provides coordination of care to children with an
emotional or behavioral health need on Mass Health and to their families. The
ICC will also provide Care Coordination to families working with the
Department of Children and Families to provide Family Support and
Stabilization Services. The children and families that work with the Walden
Wraparound Program must have a deaf member.
REQUIREMENTS:
• A Master’s Degree in Social Work, Counseling or other clinical related
field. A BA in a human service field plus five years experience will be
considered.
• Must have knowledge of the Deaf community, language and culture.
• Flexible work hours are required.
• Strong ASL and English skills (written or spoken) preferred.
• A valid drivers license and a clean driving record as determined by
Human Resources is required.
• Second language fluency in American Sign Language or ability to
demonstrate progressing ASL skills.
FUNCTIONS AND RESPONSIBILITIES:
Works with youth and family to determine medical necessity eligibility by completing required
assessment tools.
Works with the family to identify professional and natural supports.
Develops Care Plan Team with the family and the youth.
Regularly meets with team and family to establish goals and tasks.
Works from a strengths based and family centered perspective.
Liaisons with collateral agencies, schools, doctors etc.
Completes required paper work in compliance with Mass Health standards.
Works well independently and as part of a team.
